Affinity’s Reframe Youth Service provides a community based acute respite residential service and day programme for adolescents and youth who are resident of the Auckland DHB district and under the care of community mental health teams. The service is aimed at youth aged 16 to 24 years however clients between 13-15 years can be considered on a case-by-case basis.  The service is an early intervention service that provides individually tailored acute respite options to adolescents and youth who are experiencing an acute deterioration in their mental state.

 

Affinity Services takes a flexible and holistic approach using a range of recovery tools to support service delivery including the Strengths Case Management approach and Wellness Recovery Action Planning (WRAP). The Strengths approach focuses on the inherent strengths of the person to assist recovery rather then on their perceived deficits. A Strengths perspective lends itself to working flexibly with a wide range of clinical and support interventions.

 

Affinity works in partnership with and complimentary to ADHB Mental health clinical services, to ensure people receive a comprehensive range of services. Work with service users will be based on sound needs assessment, planning and regular review of progress. In keeping with best practice, care will be taken to ensure that people’s autonomy and independence is maximized at all times, and that clinical and support

 

 

Affinity’s Reframe Youth Service’s use a strengths based, early intervention approach to actively support young people in their recovery. The Respite Service’s aim is to minimise the need for hospitalisation by reducing stress levels, enhancing wellness and strengthening young people’s ability to manage their mental health within the community.

 

WHAT DO WE OFFER

We work as a team to provide:

  • 3 Residential beds for short-term stay
  • 3 additional day programme places
  • Support staff 24 hours per day, seven days a week with clinical FTE available between 11am-7pm Monday to Friday. 
  • A homelike, safe, supportive and youth-orientated environment for young people aged 16 – 24,
  • A structured Day Programme including; self awareness, accessing community resources, art and creative writing, communication skills, relaxation techniques, wellness management and recreation,
  • Staff including peer support, who value young people and believe in their recovery.

  • After Hours a referring clinician or Community Acute Service (CAS) is able to contact Reframe directly to discuss a referral
